When requested by the parents and/or the student, a different given name or even last name may be used for roll call purposes and for daily interaction and dialogue with the student. However, the legal name is the only one that remains in use on all official school records.

LEGAL NAME

A student's legal name will be established by the birth certificate or other legal document furnished by the parent when a student enters school for the first time in kindergarten or the first grade.

The school will use the legal name on the principal's report (computer print-out), the cumulative folder, and the teacher's grade book every year that student is in school.

When a parent requests that the school record and use a nonlegal name, the following shall apply:

1. The student's legal name shall be recorded on the principal's report (computer print-out), the cumulative folder, and teach- er's grade book. The teacher shall enter in pencil after the legal name, the name the parent wishes the school to use.

2. All school personnel will call the student by the name the parent wishes the student to be called.

3. The counselor should inform the parent, teacher, and student, that the grade card printed from the principal's report will contain the student's legal name. The teacher should make every effort to be discreet not to embarrass the student when distributing grade cards.

4. When conferring with parent regarding this matter, they should be notified that they may petition the court to change a minor's name without legal adoption, or that they may legally adopt the student and change the student's name.

5. When a student has had a legal name change, a copy of the legal document should be placed in the cumulative folder and the records changed to the student's new legal name begin- ning on that date; i.e., legal name change, legal adoption, or marriage
