STUDENT INSURANCE

The District shall consider various plans of student insurance available and may approve one plan to be offered throughout the District with none of the cost to be paid by the District. The District will request proposals every three or more years as determined by the Director of Risk Management, Executive Director of Athletics, and Executive Director of Purchasing. Bid specifications will be developed by Risk Management with the assistance of the Depart-ments of Purchasing and Athletics. Proposals will be evaluated by all three departments and the recommendation will be based on cost, benefits, and the company's ability to provide service.

The accident insurance will be available to students at a minimum cost. Participation in the program is strictly voluntary. The policy will provide coverage for accidents which occur at school or while traveling to and from school. A 24 hour policy will be available at additional cost.

The enrollment of the voluntary program will be conducted at the beginning of each school year. The communication will be coor-dinated with the Department of Athletics and the Department of Pupil Personnel Services. Students in athletics will be required to participate in the accident insurance program or the parent/legal guardian must sign a waiver supplied by the school district. Schools will maintain a record of their enrollment. Parents and students shall be notified that the Director shall not be responsible for costs of treating injuries or assume liability for any other costs associated with an injury at school or at any school related function. Student insurance is provided for the student's benefit, and the school acts as a service agent only. The schools shall receive no proceeds for this service and shall not be responsible for claims. Risk Management will provide claim forms and filing information to the school offices. Students should obtain claim forms from their individual campuses and should send claims directly to the insurance company.
